PATIENT DATA PROTECTION CLAUSE
In accordance with the established in Aticle 13 of the (EU) Regulation 2016/679 of the European Parliament and council of the 27th of April 2016 relative to the protection of physical beings and the respect of the handling of their personal details and the free circulation of said details, from now or known as GDPR, we hereby inform you that your details will be treated by:
The requested details of personal character are collected with the goal of administration management and to provide health and assistance services in the choosen hospitals as well as for prevention, medical diagnosis and the management of prior appointments. These treatments are legitimated for the execution of a contract in which you form a part of, or alternatively, with your given consent. If you oppose to provide the necessary details, with the exeption of those that you are legally obliged to give, for the purposes mentioned earlier, this could cause an impediment to provide said services.
However, PHS ESCARE GROUP S.L. reserves the right to treat said details outsside of the previously mentioned terms if and when it is necessary for reasons of public interest within the public health invironment such as protecting the vital interests of people.
The personal details with which you have provided us with could be communicated, whenever necessary, to the following categories of destinations:
In general terms, no international transfer of your details is carried out in the communicztions previously mentioned. However, it is possible that, in determined circumstances, the need may exist to carry out a communication of your details to third parties located outside of the European Economic Area: in these cases the required measures will be adopted by Chapter V of the GDPR to guarantee the protection of your rights and liberties.
We hereby inform you that you have the right to access your personal details; request the rectification of incorrect details; request their suppression when, amongst other motives, the details are no longer required for the reasons that they were collected.
Additionally, you have the right to transfer your personal details and request that we provide them, in a structured way, of common use and machine readable, to another entity who is responsible for the treatment, except in cases contemplated as exceptions in the GDPR.

Also if you are going to be hospitalized, we request you consent to communicate with your family or other representatives your situation and presence in the hospital center. And if you agree, we also request your consent to inform them of your state of health and recieved medical care;
